Physical, Dental, Eye Exam Requirements


Students new to Illinois, entering the 6th grade or students planning to participate in sports will need a physical examination completed within the past year. Copies of the required physical examination form, ILLINOIS DEPT. OF HUMAN SERVICES, CERTIFICATE OF CHILD HEALTH EXAMINATION, are available from the school office, from the nurse, or through the link below. Parents/guardians please complete the Health History Section, sign and date it before seeing your health care provider.

Every Middle School student must show proof of completion of having received the Diphtheria/Tetanus/Petussis series including Tdap booster, Polio, MMR, Varicella, Meningococcal  and Hepatitis B series. If any series is incomplete, a schedule for completion must be obtained from your healthcare provider for your child to attend school.

6th grade students MUST have a dental examination dated after November 15, 2016 on file with the school by May 15, 2018. Copies of the required dental examination form, the State of Illinois PROOF OF SCHOOL DENTAL EXAMINATION FORM, are available from the school office, from the nurse or through the link below.

All students new to the state of Illinois must show proof of having received a vision examination from a licensed optometrist or opthamologist. This completed form is due within 30 days of school enrollment. Copies of the required vision examination form, the State of Illinois EYE EXAMINATION REPORT, are available from the school office, from the nurse or through the link below.
