School Health Notifications
|The State of Illinois Public Act 099-0843 requires schools to request parents of students with asthma to provide an Asthma Action Plan from the student's healthcare provider. This action plan will be kept on file with the school nurse. This Action Plan needs to be submitted at the start of each school year.
**2016-2017 HEALTH REQUIREMENTS**
All students entering 6th
grade must show proof of having received one dose of meningococcal
conjugate vaccine (MCV) or commonly known as the meningitis vaccine. All
12th grade students will be required to show proof of having
received 2 doses of this vaccine, or one dose only if first one is
received on or after the 16th birthday.
In addition to this new vaccination requirement, all entering 6th
grade students will need to show proof of having received two doses of
Mumps, Measles, Rubella and Varicella (Chickenpox) vaccines as well as a
Tdap booster vaccine.
All 6th grade students MUST have a dental examination dated after November 15, 2015 on file with the school by May 15, 2017.

Vision and Hearing Screenings

8th grade students, students receiving special education services, and
new students to the district are required to have vision and hearing
screenings. These screenings are done throughout the school year by the
school nurse at MMS. A referral will be mailed to you if your child is
in need of further follow-up. Please contact Mrs. Henson if you have
screening is not a substitute for a complete eye and vision evaluation
by an eye doctor. Your child is not required to undergo this vision
screening if an optometrist or ophthalmologist has completed and signed a
report form indicating that an examination has been administered within
the previous 12 months. (Section 27-8.1 of the School Code)
